Case Manager - follow-up and outcomes (Hartford, CT)

Bluepeak Basin Civic Services provides shared administrative services to local governments around Hartford, and we need a Case Manager to lead our follow-up and outcomes. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for permits. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with permits. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in Hartford, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
