Case Manager - follow-up and outcomes (Hartford, CT)

Silverline Hollow Health runs clinics and patient services around Hartford, and we need a Case Manager to lead our follow-up and outcomes. We are busy and kind, and we look after each other. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for referrals. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with referrals. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Hartford, with some flexibility after the first few months. • We reply to every applicant.
