Case Manager - follow-up and outcomes (Fresno, CA)

We are growing our team at Fernwood Loom Early Learning Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. Children come first and so does looking after the teachers. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for licensing records. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with licensing records. • Bilingual English and Spanish. Working arrangement • Contract for three months, with clear deliverables from the first week. • On site in Fresno. • We reply to every applicant.
