Case Manager - follow-up and outcomes (Fresno, CA)

Ironwood North Learning Group is an education provider in Fresno serving students and families, and we need a Case Manager to lead our follow-up and outcomes. You would join a small organisation of about 29 people based in Fresno. What you will do • Help with applications and appointments for school calendars. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with school calendars. • Bilingual English and Spanish. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Fresno, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
