Case Manager - follow-up and outcomes (Fresno, CA)

Lumen Birch Care Services Group supports families and individuals in Fresno with personal care and coordination, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for client records. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with client records. Working arrangement • Full-time, Monday to Friday. • On site in Fresno. • Applications are reviewed on a rolling basis.
