Case Manager - follow-up and outcomes (Fresno, CA)

Greenridge Bridge Care Services Group provides in-home and community support in Fresno. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Help with applications and appointments for care plans. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Part-time, about 16 hours a week, spread across three days. • On site in Fresno. • We will tell you what to expect at each step of the hiring process.
