Case Manager - follow-up and outcomes (Fresno, CA)

Lumen Wilder Early Learning Group is looking for a Case Manager. The job centres on follow-up and outcomes for enrolment, and it would suit someone who likes steady, practical work. Children come first and so does looking after the teachers. What you will do • Help with applications and appointments for enrolment. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with enrolment. Working arrangement • Full-time, on a regular day shift. • On site in Fresno, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
