Case Manager - follow-up and outcomes (Fresno, CA)

Highbridge Loom Learning Group operates schools and learning programmes around Fresno. We are hiring a Case Manager to take on follow-up and outcomes. Collaboration between teachers and staff is strong. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for family communication. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • On site in Fresno. • We will tell you what to expect at each step of the hiring process.
