Case Manager - follow-up and outcomes (Detroit, MI)

Fernwood Glen Health provides primary and specialty care in Detroit, and we need a Case Manager to lead our follow-up and outcomes. We are busy and kind, and we look after each other. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for patient education. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with patient education. Working arrangement • Full-time, Monday to Friday. • On site in Detroit. • We will tell you what to expect at each step of the hiring process.
