Case Manager - follow-up and outcomes (Detroit, MI)

Ironwood Basin Health runs clinics and patient services around Detroit. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 74 people based in Detroit. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for patient scheduling. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling. Working arrangement • Part-time, about 28 hours a week, with flexible days. • On site in Detroit, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
