Case Manager - follow-up and outcomes (Detroit, MI)

Lumen Ashton Civic Services Group works with county and city offices around Detroit, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for permits. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with permits. Working arrangement • Part-time, about 20 hours a week, with flexible days. • On site in Detroit, with some flexibility after the first few months. • We reply to every applicant.
