Case Manager - follow-up and outcomes (Des Moines, IA)

Nova Granite Community Partners is looking for a Case Manager. The job centres on follow-up and outcomes for volunteer shifts, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for volunteer shifts. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with volunteer shifts. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in Des Moines, with some flexibility after the first few months. • We reply to every applicant.
