Case Manager - follow-up and outcomes (Des Moines, IA)

Bluepeak Vale Community Partners Group is a nonprofit in Des Moines supporting families with food, housing and job help, and we need a Case Manager to lead our follow-up and outcomes. We are small enough to know everyone's name. What you will do • Help with applications and appointments for volunteer shifts. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with volunteer shifts. Working arrangement • Full-time, with a steady weekday schedule. • On site in Des Moines. • We reply to every applicant.
