Lumen Fern Care Services is looking for a Case Manager. The job centres on follow-up and outcomes for caregiver training, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for caregiver training. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with caregiver training. Working arrangement • Contract for four months, with clear deliverables from the first week. • On site in Des Moines. • Applications are reviewed on a rolling basis.
