Case Manager - follow-up and outcomes (Des Moines, IA)

Redstone North Community Partners is a Des Moines charity working with residents on basic needs and opportunity. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 46 people based in Des Moines. What you will do • Help with applications and appointments for grant reporting. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with grant reporting. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, spread across three days. • On site in Des Moines, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
