Case Manager - follow-up and outcomes (Des Moines, IA)

Ironwood Community Partners Group runs community programmes across Des Moines. We are hiring a Case Manager to take on follow-up and outcomes. You would join a large organisation of about 609 people based in Des Moines. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for grant reporting. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with grant reporting. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in Des Moines. • We will tell you what to expect at each step of the hiring process.
