Highbridge Oak Health is looking for a Case Manager. The job centres on follow-up and outcomes for patient scheduling, and it would suit someone who likes steady, practical work. You would join a small organisation of about 25 people based in Denver. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for patient scheduling. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling.
