Case Manager - follow-up and outcomes (Denver, CO)

Lumen Sparrow Care Services is a social-care provider serving older adults and people with disabilities around Denver. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 46 people based in Denver. What you will do • Help with applications and appointments for care plans. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Denver. • We reply to every applicant.
