Case Manager - follow-up and outcomes (Denver, CO)

Fernwood Granite Property Management manages apartment communities around Denver. We are hiring a Case Manager to take on follow-up and outcomes. Residents are neighbours, not numbers. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for rent collection. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with rent collection. Working arrangement • Full-time, on a regular day shift. • On site in Denver, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
