Case Manager - follow-up and outcomes (Dallas, TX)

We are growing our team at Lumen Meridian Care Services and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Help with applications and appointments for client records. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with client records. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Dallas. • Applications are reviewed on a rolling basis.
