Case Manager - follow-up and outcomes (Columbus, OH)

Clearpath Haven Property Management is a Columbus property manager with leasing, maintenance and resident services, and we need a Case Manager to lead our follow-up and outcomes. You would join a small organisation of about 41 people based in Columbus. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for maintenance requests. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with maintenance requests. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, spread across three days. • On site in Columbus. • We will tell you what to expect at each step of the hiring process.
