Case Manager - follow-up and outcomes (Columbus, OH)

Fernwood Meadow Community Partners Group runs community programmes across Columbus. We are hiring a Case Manager to take on follow-up and outcomes. Flexibility and kindness are expected. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for client intake. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with client intake. Working arrangement • Full-time, with a steady weekday schedule. • On site in Columbus. • We will tell you what to expect at each step of the hiring process.
