Case Manager - follow-up and outcomes (Columbus, OH)

Highbridge Umber Health runs clinics and patient services around Columbus. We are hiring a Case Manager to take on follow-up and outcomes. You would join a large organisation of more than 1,000 people based in Columbus. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for electronic records. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with electronic records.
