Case Manager - follow-up and outcomes (Columbus, OH)

Stonegate Bridge Civic Services supports public agencies in Columbus with budgeting, records and programmes, and we need a Case Manager to lead our follow-up and outcomes. You would join a large organisation of more than 1,000 people based in Columbus. What you will do • Help with applications and appointments for procurement. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with procurement. Working arrangement • Full-time, on a regular day shift. • On site in Columbus, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
