Case Manager - follow-up and outcomes (Columbus, OH)

Amberfield Sable Housing provides affordable homes and support services around Columbus. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for repair requests. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with repair requests. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • On site in Columbus. • We will tell you what to expect at each step of the hiring process.
