Case Manager - follow-up and outcomes (Colorado Springs, CO)

Summit Clear Care Services is a social-care provider serving older adults and people with disabilities around Colorado Springs, and we need a Case Manager to lead our follow-up and outcomes. Teams support each other, especially on hard days. What you will do • Connect clients with partner agencies. • Help with applications and appointments for licensing. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Full-time, with a steady weekday schedule. • On site in Colorado Springs. • We reply to every applicant.
