Case Manager - follow-up and outcomes (Cleveland, OH)

We are growing our team at Rivermark Meadow Learning Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for enrolment. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with enrolment. Working arrangement • Full-time, with a steady weekday schedule. • On site in Cleveland. • Applications are reviewed on a rolling basis.
