Case Manager - follow-up and outcomes (Cincinnati, OH)

Redstone Junction Community Partners is a Cincinnati charity working with residents on basic needs and opportunity, and we need a Case Manager to lead our follow-up and outcomes. You would join a large organisation of more than 1,000 people based in Cincinnati. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for benefits enrolment. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with benefits enrolment. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • On site in Cincinnati. • Applications are reviewed on a rolling basis.
