Case Manager - follow-up and outcomes (Cincinnati, OH)

Northwind Sable Early Learning is an early-learning provider in Cincinnati. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 118 people based in Cincinnati. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for classroom supplies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with classroom supplies. Working arrangement • Full-time, on a regular day shift. • On site in Cincinnati. • We will tell you what to expect at each step of the hiring process.
