Case Manager - follow-up and outcomes (Chicago, IL)

Greenridge Bright Community Partners is a nonprofit in Chicago supporting families with food, housing and job help. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 49 people based in Chicago. What you will do • Connect clients with partner agencies. • Help with applications and appointments for donor records. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with donor records. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • On site in Chicago. • Applications are reviewed on a rolling basis.
