Case Manager - follow-up and outcomes (Chicago, IL)

Bluepeak Corner Property Management looks after rental homes across the Chicago area, and we need a Case Manager to lead our follow-up and outcomes. You would join a small organisation of about 41 people based in Chicago. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for vendor invoices. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with vendor invoices. Working arrangement • Full-time, Monday to Friday. • On site in Chicago. • We will tell you what to expect at each step of the hiring process.
