Case Manager - follow-up and outcomes (Chicago, IL)

Highbridge Willow Learning Group is looking for a Case Manager. The job centres on follow-up and outcomes for learning materials, and it would suit someone who likes steady, practical work. You would join a small organisation of about 161 people based in Chicago. What you will do • Help with applications and appointments for learning materials. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with learning materials. Working arrangement • Full-time, on a regular day shift. • On site in Chicago. • Applications are reviewed on a rolling basis.
