Case Manager - follow-up and outcomes (Chicago, IL)

Bright Willow Community Partners is looking for a Case Manager. The job centres on follow-up and outcomes for client intake, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for client intake. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with client intake. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in Chicago. • Applications are reviewed on a rolling basis.
