Case Manager - follow-up and outcomes (Charlotte, NC)

Lumen Field Community Partners is a Charlotte charity working with residents on basic needs and opportunity, and we need a Case Manager to lead our follow-up and outcomes. You would join a small organisation of about 79 people based in Charlotte. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for volunteer shifts. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with volunteer shifts. Working arrangement • Part-time, about 16 hours a week, spread across three days. • On site in Charlotte. • Applications are reviewed on a rolling basis.
