Case Manager - follow-up and outcomes (Charlotte, NC)

Lumen Iron Property Management looks after rental homes across the Charlotte area. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 24 people based in Charlotte. What you will do • Connect clients with partner agencies. • Help with applications and appointments for vendor invoices. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with vendor invoices. Working arrangement • Full-time, on a regular day shift. • On site in Charlotte, with some flexibility after the first few months. • We reply to every applicant.
