Case Manager - follow-up and outcomes (Charlotte, NC)

Clearpath Timber Community Partners is looking for a Case Manager. The job centres on follow-up and outcomes for benefits enrolment, and it would suit someone who likes steady, practical work. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for benefits enrolment. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with benefits enrolment. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • On site in Charlotte. • We will tell you what to expect at each step of the hiring process.
