Case Manager - follow-up and outcomes (Charlotte, NC)

We are growing our team at Clearpath Union Care Services and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 11 people based in Charlotte. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for care plans. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Part-time, about 24 hours a week, mornings. • On site in Charlotte. • Applications are reviewed on a rolling basis.
