Case Manager - follow-up and outcomes (Charlotte, NC)

Summit Loom Property Management is a Charlotte property manager with leasing, maintenance and resident services, and we need a Case Manager to lead our follow-up and outcomes. Residents are neighbours, not numbers. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for maintenance requests. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with maintenance requests. Working arrangement • Part-time, about 16 hours a week, spread across three days. • On site in Charlotte, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
