Case Manager - follow-up and outcomes (Charlotte, NC)

We are growing our team at Lumen Keystone Care Services and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 139 people based in Charlotte. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for caregiver training. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with caregiver training. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Charlotte, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
