Case Manager - follow-up and outcomes (Charleston, SC)

Ironwood Umber Health runs clinics and patient services around Charleston, and we need a Case Manager to lead our follow-up and outcomes. You would join a large organisation of about 265 people based in Charleston. What you will do • Help with applications and appointments for referrals. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with referrals. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Charleston, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
