Case Manager - follow-up and outcomes (Charleston, SC)

Bluepeak Corner Housing provides affordable homes and support services around Charleston. We are hiring a Case Manager to take on follow-up and outcomes. You would join a large organisation of about 538 people based in Charleston. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for maintenance stores. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with maintenance stores. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Charleston. • Applications are reviewed on a rolling basis.
