Case Manager - follow-up and outcomes (Charleston, SC)

We are growing our team at Ironwood Pike Property Management and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 143 people based in Charleston. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for rent collection. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with rent collection. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in Charleston. • We reply to every applicant.
