Case Manager - follow-up and outcomes (Buffalo, NY)

Bluepeak Ashton Civic Services supports public agencies in Buffalo with budgeting, records and programmes. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 13 people based in Buffalo. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for public records. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with public records. • Bilingual English and Spanish. Working arrangement • Contract for nine months, with the possibility of becoming permanent. • On site in Buffalo. • We reply to every applicant.
