Case Manager - follow-up and outcomes (Buffalo, NY)

We are growing our team at Greenridge Meadow Housing Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. Housing is our mission and residents are our neighbours. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for repair requests. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with repair requests. Working arrangement • Part-time, about 16 hours a week, afternoons. • On site in Buffalo. • We reply to every applicant.
