Case Manager - follow-up and outcomes (Buffalo, NY)

Highbridge Wilder Property Management is a Buffalo property manager with leasing, maintenance and resident services, and we need a Case Manager to lead our follow-up and outcomes. You would join a large organisation of about 917 people based in Buffalo. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for unit turnovers. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with unit turnovers. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, spread across three days. • On site in Buffalo, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
