Case Manager - follow-up and outcomes (Buffalo, NY)

Lumen Bright Housing provides affordable homes and support services around Buffalo. We are hiring a Case Manager to take on follow-up and outcomes. Housing is our mission and residents are our neighbours. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for lease compliance. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with lease compliance. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Buffalo. • We reply to every applicant.
