Case Manager - follow-up and outcomes (Buffalo, NY)

Harborlight Willow Property Management manages apartment communities around Buffalo. We are hiring a Case Manager to take on follow-up and outcomes. You would join a large organisation of more than 1,000 people based in Buffalo. What you will do • Help with applications and appointments for resident events. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with resident events. Working arrangement • Full-time, with a steady weekday schedule. • On site in Buffalo, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
