Case Manager - follow-up and outcomes (Buffalo, NY)

Bluepeak Kestrel Community Partners runs community programmes across Buffalo, and we need a Case Manager to lead our follow-up and outcomes. Mission matters here, and so does looking after staff. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for grant reporting. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with grant reporting. • Bilingual English and Spanish. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • On site in Buffalo, with some flexibility after the first few months. • We reply to every applicant.
