Case Manager - follow-up and outcomes (Boston, MA)

Bluepeak Rook Housing Group provides affordable homes and support services around Boston. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 39 people based in Boston. What you will do • Help with applications and appointments for maintenance stores. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with maintenance stores. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Boston, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
