Case Manager - follow-up and outcomes (Boston, MA)

We are growing our team at Clearpath Rook Housing Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. Housing is our mission and residents are our neighbours. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for waiting lists. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with waiting lists. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, with flexible days. • On site in Boston. • We reply to every applicant.
